SUBMISSION OF REQUEST FOR CONSENT

A request for consent can be submitted by a legal entity or a natural person.

The request for consent to the location of the facility/route of the infrastructure is submitted on the following forms:

• REQUEST FOR ISSUING CONSENT TO THE LOCATION OF THE FACILITY or
• REQUEST FOR ISSUING CONSENT ON THE ROUTE OF THE INFRASTRUCTURE

which are available at http://www.bh-gas.ba/ .

The applicant submits the request for consent with accompanying documentation (documentation defined in the request form and this document):

• In person at the Company’s protocol office or
• By mail

to the address:

Directorate of BH-Gas doo Sarajevo, Obala Kulina Bana St. no. 8, 71000 Sarajevo

DOCUMENTATION TO BE SUBMITTED WITH THE REQUEST

REQUEST FOR CONSENT ON FACILITY LOCATION

1. Plot data – Copy of the cadastral plan with the outlined facilty’s location – issued by the competent municipal authority, original (certified copy), not older than 6 months
2. Geodetic base of the cadastral plan of underground installations with the outlined facility’s location – issued by the competent municipal authority, original (certified copy), not older than 6 months
3. Location permit if the request for consent is submitted in the process of issuing a construction permit, Copy
4. Extract from the investment-technical documentation (description, purpose of the facility, geodetic coordinates of the facility) in hard copy and electronic form, in the case of energy and other infrastructure facility
5. Copy of previously issued consent of BH-Gas, if any
6. Other documentation about the holder of the investment and the facility, possibly other data specifying the facility, if necessary

1. REQUEST FOR CONSENT ON THE ROUTE OF THE INFRASTRUCTURE Extract from the investment-technical documentation (technical description, purpose of the project and other data specifying the infrastructure for which the request for consent is submitted) in two hard copies
2. Route / location of infrastructure in electronic dwg. format on a suitable media
3. Location permit if the request for consent is submitted in the process of issuing a construction permit, Copy
4. Copy of previously issued consent of BH-Gas, if any
5. Other documentation about the investment holder and the facility, if necessary

REQUEST PROCESSING COSTS

The applicant undertakes the obligation to pay the costs for processing the request for consent on facility location/route of the infrastructure upon delivery of the proforma invoice for the service performed by BH-Gas doo Sarajevo. The processing of the request is independent of the statement of consent/opinion (consent is issued, consent is not issued or conditional consent is issued)

The invoice for the service performed is issued after recording the payment on the bank account of BH-Gas doo Sarajevo.

If the applicant does not pay the amount according to the submitted proforma invoice, it will be considered that the request for consent has not been submitted.

REQUEST PROCESSING

BH-Gas d.o.o. Sarajevo takes on the responsibility of processing requests for consent for the location of the facility/route of the infrastructure. Delivery of consent/opinion and invoice for the service performed will be made by mail.

If the applicant does not submit complete documentation with its request or if the documentation is submitted in a format on the basis of which the request cannot be processed, BH-Gas doo Sarajevo will be required in writing to supplement the documentation.

If the applicant does not submit the required documentation, it will be considered that the request for consent has not been submitted.
